The Long-Awaited Disability Self-Identification Form is Here!!

Contractors have been eagerly waiting for the final disability self-identification form, and now the wait is over! The form was approved by OMB yesterday, and is located here. As of this writing, the form does not yet appear on the OFCCP website.

As a reminder, contractors are required to use this form, verbatim and in its entirety, to invite applicants and employees to self-identify as an individual with a disability. Per OFCCP FAQs, contractors are permitted to create an electronically fillable copy of the form, but that form must still display the OMB number and expiration date, contain the text of the form without alteration, use a sans-serif font, and use at least 11-pitch for font size. This form must be provided to applicants both pre and post offer. The form must also be provided to employees at least once every five years.
